Recently got a particle test because I had high LDL while also having High HDL and Low Triclycerides. I wanted to see if I am truly going to die

Age 31

6'0"

weight 175 lbs

Basic Lipid Panel

Total- 235

HDL- 60

LDL- 158

Triglycerides- 85

Tri/HDL- 1.417

Total/HDL- 3.91

LDL/HDL- 2.633

Particle Numbers

LDL-P- 1688

Small LDL-P- 498

LDL-P size- 21.7 Pattern A

Insulin

LP-IR score- <25

It seems I am a mixed bag? LDL total particle is high, , but I am still pattern A and am on the low side of small particles

My cholesterol is 200. It was ~150 about 4 years ago.

LDL at 150.

Guess I’ll add some more greens, cut my supplements down to only a few things and up my cardio for a retest in 8 weeks.

Just adding to the discussion. I’m curious, too.

My total levels were 140-150 back in 2014.

You fit right in line with good bloodwork. Like you mentioned, HDL is high at 60 and trigs are low at 85. If your trig/HDL is lower than 2 you are golden, and 99% of that time the LDL is pattern A, which it is. You want a small number of the smaller LDL. If that ratio is over 4, then your risk becomes substantially greater.

The reason LDL is high is that LDL transports fat (trigs and cholesterol) from the liver to the cells in your body. This is perfectly normal.

This is a pretty good overall cholesterol panel. The crazy thing is you can fast for a few days, draw, and then eat ravenously for a few days....and those number will change, even on the same diet. All these numbers are showing is an energy transport system.

Question, do you drink alcohol or coffee? I hear both increase trigs....just curious.

I drink both alcohol and coffee

1-2 cups a coffee a day and usually some beers during the weekend

Not disputing anything you said.

However I am mindful of the Framingham study as cited by Dr Caldwell Esselstyn in his book Prevent and Reverse Heart Disease. He says that only one person had a heart attack in the generations long study with total cholesterol less than 150 and that 25% of those having heart attacks had a total number between 150 and 200.

My goal is to stay below 150.

For someone like me though that seems ridiculous

My HDL hovers from 60-75 usually

So that means either I should purposefully lower my good cholesterol, or I would need LDL and tri numbers low as shite

I don’t know much about this stuff, but using total cholesterol as a measure for anything seems so illogical. Why would you add a good number and bad number together? It’s counter intuitive.

For example, you go get tested on Jan 1 and your numbers are

30 HDL
100 LDL
100 tri

Total cholesterol of 150

So you would be happy

Then you test in June and your numbers are

70 HDL
100 LDL
100 Tri

Total cholesterol of 190

Nothing changed but your good cholesterol going up, yet using total cholesterol you are now more unhealthy?

I really can’t wrap my head around how “total cholesterol” is still looked at

He would be incorrec these dayst. Unless he qualified it somehow which he may. Framingham . Even with women over 50, the higher TC of 200+ was found to be protective. The keeping TC low is only ~20 years behind the times though. So do whatever makes you happy.

High cholesterol is linked to longevity.

Cholesterol is an essential component for the creation of cell membranes and hormones. You have to remember your body makes like 3,000 units of cholesterol a day. LDL and HDL are not cholesterol, they are just proteins that transport cholesterol throughout your body. If one of those numbers is funny it is probably because there is something wrong in your body and cholesterol is being used to fix it.

It is. Your LDL could be lower, but you Tri/HDL levels are great.

How do you increase the good cholesterol? My bad is low, but so is my good. I have cut out almost all fats and oils with a plant based diet. I recently started an exercise regimen with trainer hoping that along with weight loss will improve the good/bad ratio.

That's all they told you? It didn't list the actual number?

No. Just that it’s under 25

Which seems is a good sign

It definitely is good news, but I'm surprised you didn't get the actual number. There's a big difference between 1 and 24. I think I got a 4 on my last one. If I could have only one blood marker, this would be the one I would select.

Peter Attia - Straight Dope on Cholesterol Series....long read
Dave Feldman

I'd suggest reading/listening to all of Peter's info, as he is incredibly bright and anytime I listen to him on a podcast I instantly feel like an idiot. Dave has a good entry-level explanation and basic function.

Esselstyn is still going strong and he’s not been incorrect. If anything many other cardiologists have started agreeing with him.

The studies show Esselstyn, Ornish, Kahn, Ostfeld, etc are correct.

High cholesterol is healthy..gtfo with that nonsense.

Ornish - you mean the guy who lacks controls on his studies? Then claims that is the only diet that works? That Ornish?

Kahn - The man he has out and out lied to defame individuals on Twitter. He’s a walking ad hominem - I’m definitely hooking up with him. /s

Look, I’m not going to get into the diet bullshite. Frankly it isn’t fun, gets people interested in cholesterol confused, and the conversation usually developes into a shitshow as we argue round and round. This is why I provided information to learn more, and not promote some damned diet bullshite. Do the individuals I linked to have a certain group they revolve around? Sure, however, their information is solid. I’m so fricking sick of the disagreements and two-bid utter nonsense spewed by all sides. Even the low carb subs on reddit are becoming a shitshow.

"Healthy" LDL Cholesterol on the Prison Food Diet | Dave Feldman

Feldman cholesterol challenge sounds to be pretty revealing. Apparently no one has been able to send him any studies showing high HDL and high LDL who have high rates of cardiovascular disease if triglycerides are low. But really it makes sense considering HDL and LDL are just proteins.

I need to watch this and his TG video just released. I also need to compare this to the disagreement he and Attia had over LDL. If only there were 8 days in the week...
