I always bench in a power rack. I used to bench at a gym with no safeties and always felt scared to go heavy.

The Smith machine sucks. It puts you in a strict linear path and does not require any stabilization muscles. The barbell bench is one of the key compound exercises that cannot be completely replicated by dumb bells or machines. Sounds like a lot of people are going too heavy without a spotter and i would guess have poor form.

Gonna disagree. I rotate it in pretty much for Inclines only though. Not saying it is a staple exercise because honestly I am pretty well all db's but it is a good change of pace exercise. To each is own.

I like the smith machine for decent amount of exercises.

depends entirely on goals. if you are looking to build the most amount of muscle...then after the novice/early intermediate phase, smith is actually better.

Most people aren't power lifters. For 98% of gym goers who are just focusing on fitness and building a little muscle the flat barbell bench is a complete waste of time. The mechanics of it combined with heavier weights is a recipe for disaster. There is no reason to even do it there are plenty of other exercises to choose from with far less stress and risk of tearing/pulling something.
