First one that blew me away was Super Mario Bros 3. It was such a huge leap and in the same console cycle. Zelda: ALTTP was the next one.
And then Metal Gear Solid easily destroyed both in sheer awe factor from start to finish. Closest any other has come is Baldur’s Gate II: Shadow of Amn and a distant 3rd is The Witcher 3 and those are all time greats in their own right.

SM64 is a solid choice for blowing my mind again.
So was Everquest, honestly. One of the most awe-inspiring games if you were in during the launch. I was.

Adventure for Atari 2600 was my first open world game and it blew my mind. Might have been my first console game ever.
Also had a few games from Epyx for the C64 like mission impossible and summer games. The movement of platform characters was a huge leap from Atari 2600 icons.
Dragons Lair came out in 83 and was just printed animation on a digital loop, but it showed me the future of gaming.

Resident Evil 2 blew me away as a kid. Pretty sure it gave me nightmares too.

A couple I can remember
Goldeneye was pretty insane
Metal Gear Solid was just remarkable i loved it
Grand Turismo series always got me
Diablo 2 is my favorite game all time
But I think either GTA 3 or Red Dead Redemption were the truly "holy shite" moments.

I saw Wolfenstein 3D being played at CompUSA. I immediately got the shareware first episode and started saving money for a Sound Blaster to get the full experience.

I was gonna go with Ocarina of Time bc it was the first game that I replayed as soon as I beat the game, several times.

This might be trite, but Halo was the first game that ever really blew my mind. The gameplay was decent enough, but the scale of the world is what impressed me the most. The first time I played the Assault on the Control Room level was so much fun.

Rather than a first, how about the short list of games that blew my mind. It's rare for a game to do it, normally takes the birth of a new genre.

Resident Evil 2 (the original, played it before I played 1)
Metal Gear Solid
Star Craft
Grand Theft Auto III
Dark Souls
There are more I could list but these are probably the top video games that left me blown away, because they were not only absolutely awesome, but also unlike any other game I had ever played before up till that time. They all pushed the boundaries of what a video game could be like.

Original Pokémon was the first time I spent that much time on a game. Tony Hawk was my first Playstation game. I remember seeing Jedi Outcast on PC and thought the graphics were amazing.

As a teen in the 80s, Dragon's Lair was unlike any other arcade game at the time(and for years after).
As an adult, Vice City. The radio stations, alone, made that an innovative game. But there was so much more that was new in that game that made it incredible.
