Premium is $49.99
All DLC is included. Maps being the obvious selling point but of course you get additional weapons and camo and other goodies that you can unlock from assignments.
ETA: I think the DLC is $15 if you don't have premium though. Premium is totally worth it IMO, but I play pretty much every night

Premium is $50. Close Quarters was around $15 by itself.

I remember watching Back to the Future 2 like in 86 or whenever it came out and I thought "Man, I can't wait for that hover board and those shoes to come out!"
