Steak n Shake has a rendition of the Chili Mac (with spaghetti I think). They offer the 3/4/5-Way. Not a bad rendition.

I love Cincinnati Chili/Chili 5-way. Their chili has different spices than a traditional chili, and you can buy cans of Skyline chili online. Don't knock it til you try it! As mentioned prior Steak N' Shake has an OK version; I get it at the Covington location sometimes

Chili John's is who everyone is thinking of on Food Network. They opened in Green Bay in 1900 serving chili over spaghetti. They have a 2nd location in Burbank, CA that was opened in the 40s. The chili/mac is what they're known for and the guy that started it was a Lithuanian immigrant.
