Goes back to pneumonic plague days. Virtually 100% fatal and very contagious, spread by sneezing. The God Bless you indicated a worry that anyone sneezing was about to be dead.
So, what you want folks to say to you is, "May you get plague and die."
Can't imagine why you want that. Plague is a bitch.

Did all the people saying god bless you determine if the sneezer went to heaven or hell?
Had a professor get irritated when someone said bless you ten times to someone sneezing and he finally said “HES NOT GOING TO HELL IF YOU DONT SAY BLESS YOU!”
