well this applies to every specialty

but yeah in general we hate flaky reports as much as the next guy....luckily my partners are usually very good about putting the nickel down

otoh, some rads want to swing for the fences much too often, and those are the ones that tend not to last as long

There was a radiology defense expert in S Florida that used to show up to court in a white coat and stethoscope. Until he got Boom roasted.

Ok, here is the question: CT scan of lung result,

Lung RADS Category 4X - suspicious consultation advised, next steps Chest CT, tissue sampling or PET

Clinically or potentially clinically significant non lung cancer finding. (What the frick does “non lung cancer finding” mean? Thanks

Then it goes on to describe, a nodule here and there, one mediastinal lymph node 1.1 cm in short axis (bigger than others)

There is a solid mass in the right mid lobe that abuts the right heart border and loss of normal fat plane of Rt heart border.

I am not a lungRADS reader myself, but my understanding is that 4x is used to increase suspicion about a nodule. Bottom line, this one needs PET and subsequent biopsy.

A node of that size could really go either way...PET will sort it out.

A clinically significant non lung ca finding would be something of adequate importance to be mentioned in the report, or warrant some further action, but not cancer. Like an infectious process.

The short answer is it should be assumed to be cancer until proven otherwise by more extensive evaluation.

It potentially represents a benign process such as an inflammatory condition.

Sounds like you had a low dose scan of the lung for cancer screening. 4x is the category your findings put you in. As stated PET or tissue sampling is needed for further diagnosis. The non-emergency findings are usually something found incidentally that is not urgent, but needs to be followed up.

Most Radiologist don't no how to use much less own a stethoscope

whenever theres a contrast reaction, its a scramble to find one on the cart to listen for wheezing

Sounds like you got a lung cancer screening CT. These are reported using standardized templates.

Category 4X is given to findings that are considered lung cancer until proven otherwise (hence the tissue sampling or PET).

The non-lung cancer findings are other findings on the CT not related to the Lung RADS score.
