The cigarette flick in the Usual Suspects wasn't planned, the cigarette actually hit Stephen Baldwin in the eye but they liked the take and went with it

The ending of Last Temptation of Christ where at the moment Jesus dies on the cross the film got overexposed and bleached out.

I can't remember if it is Blazing Saddles or History of the world part 1....but it is a Mel Brooks movie.
The scene where everyone is running off the movie set and going crazy. If you look there is one guy just standing there on the corner of the street. That guy was just some bystander who wouldn't leave the area after they asked him a few times. Mel Brooks kept it because he said it's pretty funny to see all these people running except for just one guy.

Which really made no sense unless the plan was to somehow "almost" kill Pentangeli so he'd turn on Michael...which obviously doesn't add up.

not sure if it meets the criteria, but the scene in Close Encounters of the 3rd kind, when the aliens come to abduct the kid - all of the things that "come alive" in the kitchen were scripted, but the actress was not told it was going to happen - all of her panic and screaming is genuine terror.
like i said - not sure if it meets the thread criteria, but thought it was a pretty cool scene. (also my mother met Dreyfus at Bel Aire Mall in Mobile during filming since part of the film was done in Mobile - she was 5 months pregnant with me at the time)

Everyone on set is stunned as Ben Stiller repeated the same line on accident, having lost his train of thought during the scene.
David Duchovny and Stiller’s wife looked at him in disbelief. Duchovny responded “seriously?? I just told you that”. The mistaken exchange was so authentic and in Zoolander’s character they kept it in the film.

Ridley Scott did something similar for the infamous chestbursting scene in Alien.

I couldn't begin to tell you the movie but one my dad showed me YEARS ago when I was a little kid was some old black and white film and there's a large gathering somewhere (almost like a school cafeteria or restaurant or somethign) and just before a man is about to pull a gun and shoot some other dude there's a kid extra sitting in the back that plugs his ears up knowing what's abotu to happen.
Not really all that "cool" but I suppose it would fit the criteria.
