L5-S1 herniated disk

Just got MRI back on a problem I have had since March. Previous diagnoses was piriformis syndrome then si joint dysfunction so it's been a long road to get to here....
Of course my doctor is out til Monday so calling on those with experience with this.
Zero back pain, glute is in a constant spasm basically since March. Had sciatica down leg but had piriformis injection and started gabapentin and have had only minor sciatica that sometimes goes about halfway down hamstring, mostly tingling. The spasms are brutal though...
MRI reads, in short, disk herniation, protrusion type. Effacement of subarticular recess. Severe left sided central canal narrowing. Severe narrowing left subarticular recess. Mild left neural foraminal narrowing. Surgical evaluation suggested.
Anyone that can tell me what I am looking at in laymens terms?

I'm assuming that's herniation auto corrected? All the narrowing is basically age related.
What does the MRI say about impingement on the nerve?

Disc is probably sitting on a nerve. Anti-inflammatory will probably help some. The only true cure is surgery. Dealing with the same issue but L3. I believe the L5-S1 is exactly what Tiger Woods has been dealing with. He finally had to have fusion surgery to get relief

No clue, but when it comes to back stuff always get a second opinion MINIMUM. Ideally get three, but back docs are the worst about misdiagnosing and running with it.

You are headed for a fusion. You sort of have the classic diagnostic criteria for that -- radiating symptoms into the hip and leg, identifiable lesion impinging on nerve roots.
A fusion is nothing to be scared of if you are otherwise healthy. They rip all that bad shite out of there, fuse the vertebrae and you end up with a solid, stable spine.
Hell, my 75 year old mother had a 3-level fusion last year and it literally changed her life for the better.

I have had 2 disk surgeries. First one was the same disks as you. The tingling sensation will eventually turn into a pain like none you have ever experienced. You will think about suicide. But, minor surgery will fix it. Hope that helps.

spinal stenosis. Go to laser spine institute in tampa. They will get bone off the nerves and relieve pain. I did the surgery last march. Try to stay off the pain meds. Take ibuprofen 800mg 3 times a day for a few days.

i really wish patients couldnt read their own radiology reports
They see "disk herniation" and "stenosis" and fixate on it and once you think something is wrong with you mentally it makes symptoms seem worse
Your doctor needs to do a good physical and neuro exam and figure out if the imaging finding match your symptoms
Almost everybody over the age of 40-50 prob has a disc herniation at some level

That's why I asked him about the nerve root impingement. He won't answer, I'm assuming there is none noted on the MRI. Many people with herniated disks are not surgical. It's all about the impingement on the nerves.

I just quoted what it said. Didn't mention worsening symptoms. Same symptoms since March.

I don't know anything about nerves on the report. I don't see mention. Is "occluding the left subarticlur recess l5s1" nerve related?
