That seems to be the common theme that everyone is leaning towards. However, when all systems fail you still have no less than 10 watchstanders with perfectly good eyeballs to look out the windows and see where the frick they're going. Its how we did it in WWII and the days before GPS and surface radar.
I asked this in another thread but didn't get an answer.
Do they have people physically looking out to sea during these shifts or do they rely strictly on electronic detection systems?

This is a theory that is starting to catch on, not sure North Korea is not involved, next we might see airplanes running into each other

There's about 10 specialities, with redundancies, that have watch at all times.

Yes, or that is what they're supposed to be doing.

Yes, there's human eyes above deck doing buddy team watches.

It just amazes me that multiple people can miss a ship the size of Rhode Island for such an extended period of time that it actually makes contact with their own the ship.

2 navy vessels in 3 months time is a sign of a problem.

Lack of discipline, not training.

If we want to get technical, 4 ships in total. One more collided with a Korean fishing vessel and another ship ran aground while trying to anchor in Tokyo Bay.

Granted.
Still, you're taking the addition of another ship out of the equation.
I can say with certainty the collective nautical knowledge of a merchant cargo ship from Asia is less than that of a USN ship.

Your faith in the young enlisted personnel of the US military is very assuring
Posted on 8/22/17 at 9:37 pm to upgrayedd
It's really not their fault. Junior enlisted will do whatever they think they can get away with. Their NCO's officers have a responsibility to make sure they do their jobs.

There's plenty of people watching the radars. There's the bridge where they plot, coordinate, and steer the ship... and then there's an intelligence room with several people in it watching radars 24/7.
