It was extremely easy for my wife to get her name changed when we got married. Don't know why it would be Amy different after.

Whelp, it appears that she could've asked for the name change be included in the divorce decree, but she didn't. It also allows for her to be able to use that very divorce decree to get it changed afterward at no cost.
Yet, I can't compel her at all to use her maiden name. So unless she decides to change it herself, she can run around using it all she wants.

File a petition and give reasons and serve the DA. Depending on the parish, filing fees will be a few hundred dollars. An attorney should not charge more then a few hundred bucks or you can do it pro se. Many clerks of court have pre-printed forms for this purpose.
