Why the hell they need to know my wife's place of employment, address, ph.#, date of birth, social, etc.?
I'm getting my freakin' shoulder looked at.
When to a dentist awhile back and one of the questions was, "Do you use alcohol or cocaine?" This was a yes or no question.
WTF?

Just draw lines through it. Hell, I'd do the same for most of those questions about me; the only ones remotely relevant are address/phone number/DOB.
