Rented a car last month in Italy and have never heard of what you’re referring to.
As I said earlier in the thread, it’s the same process as in the US when you use places like Hertz, Avis, etc. Couldn’t be simpler.

It is indeed a scam. The international "license" is not a license. I have a passport. I have a real license. I have documentation from the rental car company. I do not need a fake BS registration when I already have three that detail exactly who I am and who the car belongs to.
In the last decade I have driven many times in Europe without the fake scam "license" and the rental car company did not care one bit.

I am on your side more than not, but it’s not a scam. It’s a legal document that simply translates your American English License to Italian and many other languages. If you were Japanese it would be extremely important, as an example. Being in English it’s rare for Americans to use it. It’s for cops that can’t read english.
I love renting a car especially around the alps. Increases the amount you can see 3-4x and often tons of time savings, cost is often negligible with 2 people and cheaper with 3-4.
Get a credit card with international rental car insurance. This is extremely important, it will save you up to 50% from not having to purchase insurance. I have had to use it on a $1400 damage claim and the process was super easy. My card was USAA, someone hit me in Kent, England and I didn’t even know. We must have been parked and they side swiped the vehicle when we weren’t there.
Yes Italians drive frickin crazy, extremely offensively. Stop signs are nothing but an eyesore and recommendation to them. But it’s fun and driving around the alps is well worth it.
Renting outside of airports can save quite a bit but renting at the airport allows you to have an easy mode of departing and arriving to the airport, so juggle that.

Its not a scam at all. Was literally in the Florence rental car area on Thursday and saw someone get turned down without the IDP. Completely ruined their trip plans all in one sentence. “Sorry but we cant rent without the international license”

No international license is needed. I travel and have driven in several countries throughout Europe without issue.
