1. whistle does not re-start play, play is restarted when ball is played by player. no call.
2. yellow card to defender for delaying restart by failing to give 10 yards. (10 yards is automatic, do not have to ask for it). yellow card to attacker for "striking an opponent".
3. ball was intentionally played back to keeper by teammate. restart is indirect free kick for other team at spot where keeper picks up ball or on 6 yard box if inside he picked it up inside 6 yard box.

1. Handball. If I'm the ref I know to blow the whistle loud enough for all of the players to hear it.
2. Tell dick defender not to do it again and tell douche nozzle kick-taker the same.
3. Play on. The defender miskicked the ball so it was unintentional.

1) play-on... As someone already said whistle does not start play
2) this should also be a play-on. Defender was positioned in front of the ball before it was beginning to be kicked and the kick taker never asked for his 10.
3) I would say this last one is a indirect free kick
