Seriously - aren't you all opposed to the Readmission Reduction Program?
Do you actually think its the job of doctors to ensure you keep getting sick? In the past it may have meant more business for them, but now they actually have disincentize to do a crappy follow up job - why are you opposed to that?

DENTISTS.
I've never had any medical work in Mexico.
I can tell you with certainty that the discount you receive at a Mexican dentist's office because they don't have to worry so much about intrusive government regulation - aint worth it. Unless you like root canals.

The problem(s) I have with this is (I will use heart failure as an example, which is the #1 re-admission diagnosis):
1). "Readmission" definition is skewed. If you are admitted for heart failure and come back 25 days later for a car wreck or you get appendicitis then it counts against the doctor/hospital as a heart failure readmission.
2.) Can't change peoples behavior. You can be admitted for heart failure because you are not compliant with meds and low sodium diet and I can tune you up, PAY for a month of your medications (we DO do this for indigent patients), have a pharmacist and dietician educate you on the importance of the particular heart failure medications and diet, and have a psychologist see you to see if there are any psycho-social issues that may contribute to lack of compliance (ie. depression) and if you still are noncompliant and are re-admitted it is a penalty against me. This happens far more often than you think.
Many times it had nothing to do with lack of follow up. With that being said there are shitty docs out there but throwing everyone in the same basket and not recognizing there is responsibility on the patients part is a big problem.

So TUba thinks it's the doctor's fault if a patient with serious medical problems needs to be treated more than once.
Can't be the fact that the patient is actually....SERIOUSLY ILL.

You're only penalized for excessive readmissions. The standard is controlled for your patient types. So if you mainly treat old people your hospital is compared to others who mainly treat old people.

And there as to be a minimum number of cases. So its not like if you have 1 heart attack patient in a year and he is readmitted you are judged as 100% readmission on heart attacks.

Many. That's why only readmissions over a certain threshhold are penalized.
Further - the penalty is applied no matter where your patient is readmitted, so there is no incentive to deny readmission. If your patient tries to get readmitted and you deny it - and he goes to another hospital - you still get the penalty, except you just shorted yourself the fees for the 2nd readmission (and if you deny readmission and he doesn't make it to the next hospital, you get sued and might lose your license - so there's that)

No. I think when a hospital has significantly higher readmission rates than other hospitals that treat the same kinds of cases and patients - that hospital is probably doing something wrong. Are you telling me the opposite? I thought the right wing wanted accountability in Medicare. Is this not true anymore?

You poor soul you don't have a clue on why readmission rates are high and how they calculate them.
Let's say patient A goes to see his or her doc. Doc gives patient meds to take to prevent infection. Patient leaves and just never takes meds because they forget and it's not their priority in their life. Patient comes back to hospital just shocked they have an infection. Blame doctor for infection.

Tuba...if you are really worried about waste you would be against govt insurance. Quick anecdote I don't expect you to address. My father is an ent. He has been in practice for nearly 40 years. Only Spanish speaking ent in a very Spanish speaking area. Medicaid/care/cal requires a referral to see him...no biggie. But....When he sees a patient he is only allowed to treat them for what exact issue was referred. If a patient comes in for an ear issue and during his exam he notices a tumor in the throat.....he cannot get paid for treating it unless he sends the patient back to the primary care doc and that doc send the patient back for the throat issue. Brilliant the govt red tape.
