Ear lobes can be explained by a tight noose around the neck, which would likely come in touch with the ears. Similar phenomenon to wrestlers or MMA fighters.
The nose does look different, but it's hard to tell. Need a recent profile shot. It could be that skin droop that comes with age combined with early rigor is to blame here.

Trying to find other photos for comparison,I found that the vast majority of them he had a very similar pose in each one.
It was almost his signature move akin to women who would do the duck lip pose.

Pretty much every photo of a dead dude Ive ever seen you can readily observe that there is almost immediate change to the appearance. Maybe cessation of blood flow immediately begins slight disfiguration process. But Im no doctor. Love doctor maybe.

Looks as if he was intubated rolling in the hospital. That to me indicates the he was getting breathes via the ambu bag that the fireman is holding in his hand. It does look like he is wearing a neck collar of some sorts.
My point is, if he was intubated he was getting O2, did they call it in the ER? Or is he still intubated on a vent? Or if your really wanted to go down a rabbit hole, did they fake his death induce him with anesthesia, intubate & create a “scene” & now he is sitting on an island somewhere. He is a billionaire you know, money talks.

Good point here. And I admit Im just shooting from the hip with the medical stuff. Is it possible that EMT'S aren't empowered to call it and as a matter of procedure continue to administer O2 until a doctor or coroner authorized to make official certification of death is present?

Video footage will tell the story, but there's already a random rumor that the cameras "malfunctioned." If this is true, then it's a big middle finger from the PTB saying "yes we did it and we know you know we did it."
To do something so blatantly like this is a risk, but it's a risk the PTB felt was worth taking. Epstein must have some major dirt.

I expect that his Chanukah buddies would have the means to pull it off, but wouldn't they be better off just killing him?
For him to have a group willing to fake his death and extract him he would have to have the ultimate blackmail.
