Their “religion” calls for them to randomly explode in public. If they are willing to kill themselves in that way, why should we think they wouldn’t be willing to shoot first with nukes and hope that civilized countries moderate their response?

I wrote a long post here about the complications of Iran using nuclear weapons, from a religious point of view. LINK
Given that a fatwa hasn't been rescinded, and only a higher religious authority could rescind a fatwa anyway, the use of nuclear weapons at all by Iran is unlikely. Violating the fatwa would undermine their geopolitical goals, as it would almost certainly lead to problems with other groups of non-Iranian Shia, upon whom much of Iranian power projection rests.

Iran would not overtly lob a nuke. They would pretend they don’t have it, and pass it off to a non state actor to hit a soft target.
Then they can deny it or say it was stolen.
