I got hit a few years back while surf fishing Elmer's Island... It put a hole through my ankle between the ankle bone and the Achilles tendon.
This I learned.
1) the pain is from a toxin not from the puncture, similar to a catfish X100.
2) Get to an ER. The barb was not in me, but X-ray's showed pieces of it were and had to be washed out.
3) You will require pain meds and antibiotics.
4) Bring your wallet for something to bite down on.

Now the big lesson... The only thing that completely stops the pain is hot water... as hot as you can stand it. That's why people think peeing on it helps. I had to ride home with my foot in a bucket of hot water, the moment I took my foot out the pain came back, put it back in the bucket and the pain went away. Overall it takes about 8 to 10 hours for the toxin be neutralized.
Had a swollen foot for a month, fought crazy infection for just as long, and to this day my heel is slightly numb, it must have cut a nerve.

Hot water will relieve the pain. I have heard of using water from outboard tell-tale. (Recommend motor in neutral)

Clean immediately and seek medical attention. Especially during warmer months. Had several patients hospitalized last summer after sting ray stings with severe infections. Some very dramatic surgeries usually followed. Can usually be avoided with prophylactic antibiotics.

Look at all.the fricking idiots saying pee on it.
pee(ammonia) is for jellyfish stings. I got hit by a stingray a few years back and didn't k.ow about the hot water treatment. It is the only thing that will draw the poison out. Went a full week in Fourchon in the most amazing pain imaginable.
soak in hot water for stingrays.
