I would think fire, but things like that, drowning, mauling would make your body just shut down after a few seconds. To me the 9/11 passengers and the passengers on the suicide pilot plane a while back that spent 8 minutes knowing they were going to crash into a mountain would be bad. Falling is my main fear, so I'd probably rather fight a bear than get sucked out of plane at 30,000 feet

Bulldozer

Buried alive (hands and legs tied) with a plastic bag full of live kittens.

They will slowly claw your insides out.

If you fell out at 30000 feet you'd be unconscious from lack of oxygen almost immediately. You'd never know you died.

What was the stretching machine they used in medieval times? That would suck.

Forced to butt chug gasoline. Then have it ignited via a gas soaked towel jammed deep into the aforementioned anal canal (leaving one end out just enough to light, of course)

The tub

Known as the punishment of ‘sitting in the tub,’ the convicted person would be placed in a wooden tub with only their head sticking out. After that, the executioner would paint their faces with milk and honey; and soon, flies would begin to feed on them. The victim was also fed regularly and would end up swimming in their excrement. After a few days, maggots and worms would devour their body as they decayed alive.

People that have been resuscitated from drowning say that they went into a dreamlike state and there was no pain.
