Recipe for disaster from my experience. To each their own but I’d never put all of my wife’s money and my money into one account. Been there, done that, never ended well.
Knowing what each other spends due to joint accounts lead to a disaster, but separating accounts with the same combined income and spending habits, just ignoring the possible issues is not a recipe for disaster? How about just be functioning adults who can work together to maintain a family budget. Sure there are difficulties, but if you can't work through them and instead just keep it from each other, then you are going down a bad path.
To the OP, I can kind of see the point. If my wife joined a gym to lose weight, but kept it from me, it'd seem weird. Like 4 months in I find out she goes to the gym everyday at lunch without a hint to me...I'd think something might be fishy. It's not the money in that case, but an odd thing to not know about. I know GLP1's aren't exactly the same, but still would seem strange. Maybe not a big fight and hurt feelings, but strange.

In my experience, no. Just the opposite. She makes her money and spends it on what she wants. I make mine and spend it on what I want. We have a joint account for paying bills, groceries, gas, etc. Common items.

Marriages having separate private financial accounts is a stupid trust issue to have.
Being upset about GLP1s and being in the middle of your wife’s health maintenance issues is a trust problem that any man needs to get over. Any woman is going to be insecure to talk about weight and female health. This does not come across as stuff to get in that healthcare business.
