Thumb. I use a trackball most of the time.

ETA: I just tried it, and I'm coordinated enough to adapt pretty easily. I'm going to pay attention to my mouse necessities over the next few days, and, if I think I need it, I'm going to switch to this method.

It doesn't feel very ergonomic at all on my mouse. I tried it when I read that's how he does it. I do have a small mouse, so maybe that's why, but the scroll wheel is not in a position that works well in that configuration.
