True, this will only work when b = a + 3 for every line. It makes the value of b irrelevant.

Possible answers:
1) You add each equation to the previous result, so
1 + 4 = 5
2 + 5 = 7 + 5 = 12
3 + 6 = 9 + 12 = 21
8 + 11 = 19 + 21 = 40
or
2) You apply the pattern of both numbers increasing by 1 and the equations progressing in sequential order and extrapolate out to what it would be when you got to the last one, so
1 + 4 = 5
2 + 5 = 7 + 5 = 12
3 + 6 = 9 + 12 = 21
4 + 7 = 11 + 21 = 32
5 + 8 = 13 + 32 = 45
6 + 9 = 15 + 45 = 60
7 + 10 = 17 + 60 = 77
8 + 11 = 19 + 77 = 96
ETA: or, apply the formula (a*b)+a=X, so
(8 * 11) + 8 = 96
As others note, (a * b) + a = a * (b + 1), so
8 * (11 + 1) = 96

You left out the possibility that the answer to each line is independent of the others. That's how you can get 96 without extrapolating anything.
A x (B + 1) = C

there are three possible solutions that would be correct depending what pattern you use.
I think it's 40 as that is the most straightforward pattern.

The original provider of the problem states that only 1 person in 1000 will get the correct answer, yet based on this thread and Facebook it seems 999 people out of 1000 will produce 40 or 96.

I'm sure this has been discussed already, but doesn't it depends on what the question is asking? If it's stating that "8+11" is the next in the sequence, then the answer is different than if it's asking "keep going until you get to '8+11' and what is that answer?"
