It's usually 2-3 minutes for me. Unless it's DC training then it's 30 seconds.

I go heavy on weight and rest with pretty much everything but cables. I use machines for high reps. Pretty much the only time I go lower with weight is when I need control for a concentrate movement. I do these DB curls kind of like Arnold press where I rotate going down/up. Ridiculously good for cutting biceps. Things like that I'll go lighter but for most everything I take > 2 minute rests.

Same here. I figured the 4-5 minute breaks were needed for the type of weight those guys push.

How long have you been at 245? If you gained 60lbs of muscle mass you aren't quickly gaining much more with that workout (or any) unless you're eating a frick ton. Slow progressional gains from here on out.

A while now. I don't really have a desire to get bigger. I just push myself to get stronger. Sure, it would be nice to have 25 inch arms, but 19-20 inch arms aren't small.

Then that's cool. Good on you, man. I guess rest time is relevant to what you're trying to do.

I used to do circuits and gained a lot of strength that way but my bench kind of plateaued at 265. Was 185 at the time and gaining weight was my priority. I started focusing more on mass and less on endurance. I run a bunch for that.

Whatever works for you.

If you are on a strength program you should rest at least until your breathing has returned to normal on all sets. On work sets I believe you should rest at least 2 minutes.
