I had a CT scan for a suspected kidney stone, age 56. negative for stones and anything else EXCEPT for severely enlarged prostate. so had I the digital exam and keep an eye on PSA, (very low). Dr's said if it becomes an issue then we can talk medications etc. So far, so good.

This is good advice. PSA tests are already pretty inadequate as a diagnostic tool given the amount of things other than cancer which can elevate PSA levels...I have asked many urologists about the effectiveness of PSA testing for prostate cancer and its a lot like capitalism, it is lousy until you compare it to all the alternatives. It is the tool we have. It is entirely possible to substantially lower your PSA with all sorts of over the counter supplements and prescription drugs...doing so simply to lower PSA is a bad idea....it can mask a very serious health condition.


Speaking of tests that aren't great diagnostic tools consider the DRE (Digital Rectal Exam - the finger up the backside). I have asked every urologist and oncologist who have treated me for the last 17 years about the DRE as a diagnostic tool and everyone of them agreed it is basically something the patient expects and is not overly informative. The first one done by a trained professional can indeed indicate an enlargement and possible even a lump or lesion...subsequent DREs, especially performed by another doctor several months later, rarely add any new info which is not known from imaging etc. I have told doctors I was fine with them NOT fingering my arse and nary a one disagreed with the sentiment....its pretty useless as a diagnostic tool by all accounts
