According to the news, Trump did not know about it until after the fact. The bomb was sent over there during the Obama administration, and Trump gives military the green light to do what they decide to do.

MOABs blow up in the air and create a super intense downward directed shockwave.
The intense force of the air destroys anything around the blast zone. The shockwaves near ground zero would be fatal, and the rapid pressure changes it would cause in a cave could also be fatal. Not to mention it will collapse the entrances to the cave.
they're GPS directed and pretty accurate. Also, it's pure shock and awe. People who saw that pissed their pants. I bet some thought it was a nuke when they saw that cloud.
