I said my first prayer in I can't remember how long thinking about you and your family last night going to bed. I hope you have a good support network at home and are able to eventually find peace and comfort

This is literally the most gut-wrenching thread I've ever read on here or any message board. Maybe it's just the timing as my daughter is 4 months, but I haven't stopped thinking about this since I first saw it.

Jesus. I hope you get answers from the hospital soon. Seems like the ER doc wants to understand if there was an error.
The first questions I’d get answers for are what was the initial volume of that bag of dextrose/saline (1000ml, 500ml, etc), the time between when it was hung and when it emptied, what infusion rate was programmed into the pump, and if there was also potassium (K) in it.

Triggered as in something odd in his past or triggered as in he initiated the investigation?

Don't pressure sleeves increase the blood flow to the heart, even when used as an IV assist? Could something like this put pressure on his heart?

Not sure of the size of the first IV bag but my wife told me when they brought in the second IV bag they told her that they didn't have any bags for small children and the nurse poured part of the contents into the sink before connecting the bag. Below is what is written in the IV STARTS section.
Start Time: 13:00 NS 0.9% IV 200 ml/hr Stop Time 13:30 Amount Infused 200/0.
Start Time: 15:00 D5 1/2 NS IV 50 ml/hr Stop Time: 15:30
That's all they have written for the IV section but when he coded at 16:51 the IV and bag was still connected. They were not using a programmed pump to deliver the fluids.
ETA I can't find anything on other medications they administered but I saw with my own eyes the nurse push something down the IV line with a plunger.

She wrote the below in her report.
"Frankly, I am confused about the series of events leading to his death". "I anticipate this case will go to the medical examiner."
Then later she wrote and highlighted "making police report to HPD at this time. Waiting for investigator. (Significant).
So that would have been just a short time after he passed because the investigator contacted me that same evening (twice).
Not sure if any of that means anything.

Yes it's an urgent care clinic and when I drove by it yesterday it was still open. It's a pretty new clinic.

She is doing okay. She is active in the church community and has been receiving a lot of support through them.

Prayers to you and your wife. Very few of us can understand your grief but know that you have all of our prayers. I could provide some insight but now isn't the time. The best advice I can give if it is worth anything to you. Is there is no text book to dealing with this. A lot of people will try to help, it will be genuine and heart felt. You and your wife must find what comforts you, there is no right or wrong way to do this.

Given the abrupt nature of this event, this increases the odds that an air embolism secondary to the pressure bag was the culprit. This is a rare occurence but certainly possible. This needs to be investigated as a possibility in the death of your son.
