re: Phillips 66 in Paradis - Possible Explosions

So the parish official on channel 8 said it could burn for hours to multiple days. Sounds like the pipeline is open and is unable to be closed/damaged. Just have to let whatever is left in the pipeline between there and the next valve burn.

If you wear a red diamond logo on your Nomex you may have been.

How do you know Williams caused it? Maybe it was a contractor? Maybe it was multiple contractors in all of the accidents? But I guess you know.

One of my buddies is still missing. Former Chevron Pipeline now Phillips 66. 2 were found that got out, they think my bud didn't get out in time. Still praying.

That terrible. I hope your buddy is found safe.

Supposedly they cut corners to save money regularly, cost more in the long run everytime
In fairness, the cause of the Geismar site explosion wasn't really a cost saving measure as much as altering safe operations of the equipment.
Yeah, I know it sounds like I am putting the blame on operations, but they fricked up really bad in the situation and removed the PSV from the equation.

Fire occured at the Phillips66 operated Paradis pipeline station.
The pipeline has been isolated and remaining inline product is being allowed to burn out.
There were 6 workers on site at time of fire. 5 of the workers have been accounted for...1 Phillips66 employee remains unaccounted for
Realize un-knowing rumors are out there being spread

Ops was just running equipment that was engineered unsafe. Not sure the blame should be put on them. Should they have noticed it? Maybe so...

It is designed like every single other exchanger/reboilers. Hot side/cold side. Never open only the hot side, and especially never open it first.
Valves are locked open all the time for pressure relief reasons. Someone made the decision to close a valve that is supposed to stay open for pressure relief reasons. Someone removed the lock.
Whether it was ops or ops management multiple failures occured. Lining up only the hot side was a terrible decision that shouldnt be made by an ops manager or operator.

Any updates on the 1 missing? Did he just run home or something?

Bingo! Loss Prevention Principles must be a foreign idea to them. Each exchanger should have it's own Pressure Relief Valve if engineered properly.

I take it you didn't read the other response to the post you were responding to. There was a PSV that was taken out of service by isolating it/removing a lock that was intended to lock it open, according to that post. This is a common and approved practice. The valve is there to allow for testing/servicing/repair/replacement of the PSV without having to bleed the whole system down. That means there should have been very explicit procedures for isolating the PSV and/or removing the lock.
Hopefully Phillips has all that in place. Many years ago the had a very robust safety culture in their plants, but those cultures often don't continue across business units so I am uncertain about their pipeline group.
