OT Medical question.

This morning my mother had a very brief episode of mental confusion. She has had a TIA before and thought it was that but the ER doc didnt think so. Her BP had spiked pretty high. It is usually in the low normal range with medication.
She has been completely lucid since the one event and besides the BP, has no other symptoms except what she says is a sinus headache. Did not do any scans. He said her bloodwork looked good. Sent her home with Ibuprofen for the headache and instructions to see her internist next week. Did not seem concerned at all. He thought a viral infection might be causing the high BP. Curious to hear the thoughts of any medical pros.

Should probably add age range. But if blood work is fine, that's an excellent sign.

In her seventies. Good health and lives independently with my father. No medical issues ecsept controlled hypertension. Like I said, has had TIA. Lasttime she was in the hospital was 2 1/2 years ago for flu complications. Former smoker.

Since the TIA, has she had any other issues mentally? My mother has had a couple and often repeats things to my brother and myself that she has told us before. She also forgets details of events.

Nothing I have noticed and I see her or talk on the phone nearly every day.

Hiw high was BP?
If it was really high with neuro symptoms probably should have had a CT at least.

But to answer your question, elevated bp with confusion alone could be PRES (posterior reversible encephalopathy syndrome). Not too common, but def a thing. Should've gotten a Ct regardless unless I'm missing some other parts of the story

Not a medical guy, but that sounds like a good thing. Hopefully it was just an isolated incident that is not dangerous. I wish her the best.

Around 180/75 at the ER. She took it at home before going and it was about about 150, so maybe stress was a factor at the hospital?

TIAs usually don't cause confusion contrary to popular belief. And viruses don't cause blood pressure elevation for that matter. Now if she had some sudden word finding issues with numbness or weakness on one side of the body that would be cause for concern.
You can always ask internist for further workup on Wednesday but just be vigilant until then. But I would be more suspicious for a UTI than a TIA in this situation. BPs are frequently high in the ER or when folks are anxious so I wouldn't make too much or that.

She may have a UTI and some dehydration in the mix.

Does she drink alot? B vitamin definciency will cause this.

Yep- dehydration/UTI are very common causes of confusion in senior citizens. Although the sudden onset and relief make me wonder if that is he right track.

She felt it coming on and knew somethimg was wrong. She said she suddenly didnt know how to do the task she was doing. Then it passed and she was fine.

I would make sure she sees her primary Dr. this coming week. Did they do a urinalysis? That would have ruled out UTI. Best of luck to her.

Thanks. No urinalysis that I know of. They did do a CBC and everything was normal.

Really surprised they didn't ct scan given her bp, description of symptoms, and history. Maybe it was resolving so quickly they didn't find it necessary.
