If you're in BR go to Hope Therapy. The owner is from Nigeria where PTs have to go to medical school. My dad (who has MS) has been going to her for years. She will probably tell you surgeons love to cut unnecessarily and can rehab it.

I had a "12 to 6" anterior tear in my labrum that was arthroscopically repaired. I didn't have pain but I'm a side sleeper and my arm would pop out every so often, and I also couldn't workout or enjoy hobbies as much. Had it repaired and now I'm at around 80-90% due to my own stupidity.
That said, get it done, best advice I can give you is don't frick around with the therapy. Follow it to a T. For example, don't go riding in a boat in a rough lake while you are still in a sling
ETA: They'll give you a nerve block, which sucks in itself. Whatever you do, make sure you take your first pain pill on time. You won't want to be waiting for the pain pill to hit while the nerve block wears off. I was kneeling down in the middle of the kitchen crying from the pain because I slept through the recommended time to take my pain meds.

Small tears can heal themselves with down time. I'm talking 1/4 " or less. Larger tears usually require surgery.
I had recurring dislocating shoulders from football and baseball (pitcher). best thing I ever did was get back into weight-lifting (as long as you don't go all heavy with bench and other shoulder excercises) and taking up archery, which strengthened the muscles surrounding the shoulders. Unfortunately aside from the damage from football and pitching, I managed to have a mild case of the bends on one deep dive in the Red Sea when I worked overseas.
End result is I have now undergone arthoscopic clean outs of both shoulders, which helped, but didn't fix my shoulder issues.
I have since had BOTH shoulders replaced at age 54. Still go to the gym a couple times a week (light), still bowhunt and still do the occasional elastic band PT to help keep my range of motion.
Shoulder surgery sucks, and while it was absolutely necessary in my case (zero cartilage left on either humerus) I wouldn't wish it on a broke-damn dog....or even a bama fan.

Glad you started this topic because I am pretty much in the same position. Bone rubbing bone feels like. I deal with the pain as much as I can, but have so much that I have to take care of if it’s lengthy recovery. I take care of my parents and I have custody of my 5 year old daughter. Mom just had major heart attack and still in hospital plus dad fell and broke his wrist and had surgery last week. And we all know how much it takes with a little kid everyday.
Anyways, I take my dad tomorrow for a check up and was going to discuss with his dr. Really one of the best around. It’s all old sport injuries caught up to me. Shoulder, knee, and ankle. So at almost 38 I’m wondering if I should bite the bullet when everyone else is healthy. TIA
