Things are being done to encourage healthier living/fitness levels though. A number of insurance plans offer discounts, incentives or subsidies to cover healthier behavior / fitness levels.
A lot of it has to do with how much the employer cares about their insurance plan. My wife's last company's plan offered the following
* Subsidy for gym membership costs
* $25/m discount for insurance for any person who participates in the free wellness program which required diagnostic testing, nutritional classes and fitness/activity logging

Most insurers seem to be leaning towards "healthy discounts" instead of "unhealthy charges" for fitness.
If their covered insurance costs are reduced, the discounts pay for themselves. I'd guess all else equal people in better health/fitness perform better. I know when I'm eating/working out, I have more energy/focus at least

All of those requirements are incredibly easy to meet and can be done without changing your lifestyle or actually getting more fit. In other words they are a joke. If they were serious they would require everyone to meet a certain required BMI.

The vaccine is not free. They are $12-$18 per person that is paid for by the government. The government gets its money from all working people. They do not make it, and what they "print" is funded by money from private banks/institutions that are paid back with interest. Take a look at your check stub, no the deposit record and see how munch money is stolen from you as "Federal Tax, Medicaid, Medicare, etc."

Huh.
I am on my wife's insurance which is through the Office of Group Benefits. In other words... a HUGE group.
They offer $10/month off premium for the employee only (not for spouse or kids) if the employee does an annual checkup.
You can also get a gym membership for $25/month which considering a lot of gyms are that cheap or cheaper... not much of a deal.
That's it.
So much for a wellness program.

They are also the least important when it comes to providing care.
So if John from accounting leaves to go to another place of employment that doesn't require it... does that really impact the work the nurses do on the 5th floor?
