Make sense of this Medical Bill for me please

In July of 2019, I had a small procedure done to remove a piece of skin that had the potential to become cancerous. It was not anything serious to where I needed to be put under. And I have medical insurance through my employer.
About 3 months later, I get a bill for the pathology lab. It was like $60 and I paid it. Thought that was the end of it.
Well today I got another bill in the mail for $30 bucks from the same pathology lab. Mind you this is over 8 months since the procedure. it’s not the dollar amount, it’s the simple fact that these people can turn around bill you whatever the frick they want and we have No idea what it’s for.
Plan to call them tomorrow, but why is our healthcare so F’d like this? Sunuvabitch

You're gonna be on hold for an hour, then placed on another hold. Eventually you'll get frustrated and just pay it.

Move to buying everything cash. It is hard as frick to do, paying off your house is the biggest.
After that you can tell them to go frick themselves. They can't ruin your credit if you don't need it.

Fight it. It should be covered had the same thing happened to me a couple years back. Eventually the doctors office and insurance worked it out. You probably did not request the test therefore you should not have to pay for it.

What does your explanation of benefits say?

What does the EOB from the insurance company say?
If it says you were supposed to come out of pocket $90, then that’s your answer. Pay them and move on.
If not, it sounds like they could be billing you in error for a copay that you already paid.
This, by the way, is a good example of why you should always check your EOB before paying anything to anyone.

You should have gotten get an Explanation of the bill from your insurance company. Even if you threw it away you can look it up or call them and they'll send you another one. Make sure you look at this and understand it before fighting it or they'll just give you some bullshite answer.

Your EOB comes from the insurer, not the provider. You probably got it shortly after your procedure.
You should call your insurance company first IMO.

The frick kinda insurance doesn't provide an EOB?

Let them know that you didn't receive that and will not pay until you do.
You should receive an itemized statement from the billing service as well.

Had the same kinda thing happen to me a few times.
You need to review what your provider negotiated charge is and what they claim.
I started to mail a copy of my insurances copy that was sent to me to stopped that BS.

Call them. They probably fricked up and will delete the charge. It’s happened with me.

If I had to guess I'd say the $60 charge was your coinsurance for the procedure and the $30 was your copay for the visit.
Copay probably slipped through the cracks.
But again, an EOB would settle this quickly.

Th bill came from the provider and is not accompanied by an EOB.
OP, the bill might be from either from the pathologist or the lab if they are billing you separately.

Several possibilities :
Is this a new charge or a balance due?
If a balance possibly the insurer paid $ 30.00 less than anticipated when the original billing and co-payment transactions occurred.
It may be a partial charge which is being disputed by your insurer. Until resolved the balance due remains your responsibility. The system just generates bills at certain intervals and blindly sends them to the persons of responsibility.
Review you EOB and determine what you insurer paid. If they paid properly then determine whether the doctor properly adhered to their agreement with the insurer. They can bill whatever they want to begin with. Most agree to accept whatever they agreed to with the insurer plus your copayment. Any balance left is written off.
Could be an inadvertent oversight and they did not clear the bill once payments ere received.

I got a bill recently from 2016 when I had insurance that paid 100%. If they are too stupid to file with the insurance company in timely manner they can forget about me ever paying it.
