The color blending. The hat proportions are wrong. Sand coloration/dithering/blending (as if the algorithm got confused on what was sand and what was skin.) The skull doesn't even look like it came from one human. It's like an amalgamation of humans. Forehead has a very masculine shape/proportion, prominent brow ridge.
Just very odd looking picture overall.
