Is there anything like tiered health insurance?
I've only heard about health insurance differences in the form of HMO vs PPO and/or deductable differences.

I haven't heard about tiered coverage differences. As an example:

Say someone has diabetes tier 1 coverage means you get syringes & vials of insulin. ($4 dollar solution) Tier 5 coverage means you get everything 1 thru 4 could get, but you can also get Appleplay Bluetooth compatible insulin pump ($10k option)

Like tier 1 is for medical treatment that was available in 1970, tier 2 the 80's, tier 3 90's innovations, etc..tier 5 is the state of the art $$$ R&D treatment.

Think food. No person should starve to death, but why do people who don't pay for it deserve filet? Bread/water at your payment level.

Kind of a compromise, "free health care" but not high tier care.

It's not tiered, but every plan typically covers different things.

That’s great and all, but with Today’s pricing Tier 1 is the $10k option, Tier 5 is probably the $10,000,000 option.

I think you can get some version of this, but if it is employer sponsored you pay taxes on the premium above other tiers and Obamacare still limits what treatment and diagnostics are available. Google “Cadillac plan”

What your kind of describing is our current set up although not exactly.

What’s more interesting is tiering providers and basing co-pays on those tiers. Take labs for example:
Tier 1 might be labcorp and labquest. Those are free.
Tier 2 might be independent labs. Those have a low co-pay.
Tier 3 might be labs located at clinics or hospitals. Those have a high co-pay.

The idea is to steer members to the lowest cost providers for routine services.
