People aren't robots, just because they were trained to do it one way doesn't mean they follow their training.

Now it doesn't change the fact that someone got shot but I don't see why you wouldn't think that someone could accidentally shoot someone, there are better and very obvious examples of this. I think someone posted one the other day with the dude on the ground and the female cop comes up and very obviously shoots her gun when she did not intend to.
