Go to the gym and work tf out of your rear delts. Strong rear delts hold your shoulders in proper alignment.

Get a better desk chair. The Office Depot $49.95 pleather specials are not doing you any favors.

The Aeron or other Herman Millers seem like a huge check to write for mid-three figures, but pay for themselves after ac couple of co-pays to the chiro.

Same thing with your shoes. One or two pairs of very good shoes will pay for themselves after some doctor visits.

Don't sit all the way back in the chair, and force your abs and back to do some work, for starters.

You're going to have to remind yourself to pull your shoulder blades back for a while until it becomes second nature, so I'd suggest enlistment in the military.

Strengthen your back and lats, but also look at how your desk is organized. If your desk or chair is the wrong height, or your mouse and keyboard are too far/too close to where you sit, all that kind of stuff can drag you into slouching.
