You basically wrote the post I had been formulating in my head while reading the thread. Now with only the facts in hand I'm on the fence but I guarantee this has happened to some people by accident.
I remember one time 10 or so years ago I left work early to file some papers personally because they were so important I just didn't feel comfortable having a clerk file, even though they had never failed this task before. I was just a block away when there was an accident right in front of me where the car ahead of me was t-boned by a person that ran the red light. I was calling 911 and checking to see if both drivers were "OK" (and no I didn't give them my card but I did give them my personal number in case a witness was needed). I was out of the car maybe 5 minutes, when I got back in the car I proceeded HOME, I happen to notice the papers on the passenger floorboard, I guess they slipped off under heavy braking and turned around and with a little sprinting I made it to the clerk's office by 5:00. No, it wasn't a sleeping infant but it was an 8 figure case and if you had ask me 5 minutes before I would have sworn short of a zombie apocalypse or a nuclear strike there would be no way I would have forgotten it. I was literally freaked out for days because I had lived my life thinking I was immune to the level of "humanness".
I see some of these that are likely 100% like a parent spending 4 hours mid-day in August in a truck stop casino. Others I am not nearly as certain about.

Guardian? This kind of thing is usually blamed on a "break in routine." Did she take the child to the sitter often enough for it to even be routine? If not, how could she possibly forget that she'd loaded up the baby that morning?
Never say never, but it's hard to understand how this can happen in any scenario. Many times I caught myself turning out of the driveway toward daycare instead of toward work when I didn't even have the kids in the car with me. Not once did I turn toward work when I did have the kids in the car. The routine was ingrained: get ready for work, get in car, head toward daycare. If the routine is set, it's hard to understand how someone can drive straight to work and never realize the "daycare" destination was left out of the drive.
So sad. RIP baby boy.

It happens. People get in a rut, minds wander on the way to work, getting mentally ready to face the day.
It sucks.
Everyone has spaced where they were going, or missed an exit because they weren't focused enough in that moment, but like in your example....never once did you forget your children. That's the part I don't buy. You can space out and forget to go to the daycare on your way to work....but at some point you realize it because you see/think about your child. When my child is quiet for a long period I look back just to verify that she is sleeping.

How hard would it be to integrate a carbon dioxide sensor and a thermostat with the lights and horn of the vehicle to sound an alarm to prevent this kind of tragedy? Surely with today's technology something could be invented.

This is a very rare happening. The odds of this happening to a thoughtful, caring, sober parent are virtually non existent.

How many times a year does this happen in the USA? The odds are incredibly small that a normal parent will forget their kid for a day. So small as to be damn near impossible

Yeah, I’ve caught myself halfway to work some mornings when I was supposed to drop the kid off at daycare. And we’re usually singing Frozen together, so I can see how it could happen. That said, everytime this happens it seems it turns out it was on purpose. I’ve heard when it does happen, the smell is absolutely atrocious. I don’t buy that she didn’t realize until she got to the daycare.

My truck bings the message to check the back seat when I shut it off. I thought it would get where I would ignore it after a while, but it still bings and my mind still says check the back seat.
