For your Grandmother, a monthly plant in bloom. Just to brighten her day. Jackson and Perkins used to offer monthly deliveries of plants that would bloom for about a month.
If you know some one who gardens, from Sams Goatskin Gloves. A Hori (a great digging tool) from Amazon or Duluth Trading. The Handbags from Duluth are not very stylish, but are excellent craftmanship and will last a decade.
For a person with a woodworking shop or a car hobby, there are some large magnets on poles for dropped screws. My husband really likes the sweeper magnet for gettings screws and nails out of sawdust and planear debris.

Really depends on what you want to spend. I've given young and old mattress warming pads that they've loved once they use them. I have some cold natured folks in the family, so those types of gifts work out. I've also given some of the electric heaters, nice throw blankets, lottery tickets and good electric fans like Vornado fans. Some of those gifts don't seem to be exciting at the time, but it never fails that the recipients come back long after the fact and tell me what a great gift it was. Although they are mostly utility gifts, they are appreciated.
Good flashlights or battery operated lanterns have been good gifts. I'm giving a bunch of people the Atomic Beam flashlight this year as one gift. I love that thing. It's small so it fits on your table next to the bed, in a drawer or in your glove compartment in the car and that sucker is some bright if you want it to be. We lost power one night and I could see way down the street with that thing that a big tree had fallen down. I include the batteries with the gift. My Mom said the other day that she always loves when I find a new good flashlight type thing. The more, the better since you don't know where you'll be when the power goes out at night. They're only 20 bucks and on Amazon, I found them cheaper because I ordered a bunch at once.

She lives on [very low] social security and can't afford to spend more than like $10 on us, so buying something more than ~$20 (unless it isn't obvious) would make her feel bad. I ended up getting her a high end hand cream/body wash set that came with a spa robe. Found it on sale for $25. But it doesn't feel personal. She simply isn't "interested" or excited by much of anything
The candles we got for the previous holiday are still unburned. That's another thing - a lot of the people who have zero interests/hobbies, who don't care about food/drink, are low-limit people on our list. It would look bad (and I can't even afford) to buy a $75+ gift for someone just because I can't think of anything else.
I think that's my big gripe with holiday gift-swapping. You're buying something just to buy it.. because it's just what you do and is expected. But it isn't personal or special many times.
I have a friend I haven't seen in an entire YEAR who still goes out and buys me Christmas gifts every year even though we don't talk, never hang out, etc. Never asks if we are swapping gifts either. Will just text out of nowhere asking if we can exchange gifts 2 days from now (she just did this). Then I have to run out and buy some overpriced crap that i have no idea if she will like.

If you do know someone who gets excited about food items, I've recently discovered the magic that is food at TJ Maxx/Tuesday Morning/etc.
Tuesday Morning has a massive food aisle with cool items to put together into a basket. I think it's a good idea for college students living in dorms, or anyone who likes snacks and unique food-related items.
