They are not trying to kill their customers on purpose but their supplier’s quality assurance isn’t great and their ratios are often off.

Back in the 1990s doctors got sued for not "adequately" treating pain. The government pushed the idea that "pain is the fifth vital sign." Accreditation agencies dinged hospitals for not constantly asking about pain. The media splashed story after story of doctors undertreating pain at that time. a 1994 report noted that studies found up to 50% of acute pain was undertreated, and specifically cited physicians' fear of prescribing opioids.
