The standard is 2 weeks but there's really no rule or law governing this. If you're not in an at-will state then there could be laws stating that an employer firing someone who gave notice (regardless of what the company's notice policy is) is at fault.
During my early college years I worked at one company who tried to say managers leaving the company had to seek out, hire then train their replacements. Not sure how that worked out, I was just seasonal help during the Christmas break and didn't go back.

It depends.
My current company treats its employees like fricking gold and always has. They'd get 30 days easily if I wanted to move on and they asked for the transition time. I'd probably do 2 months, if we're being honest and I could swing it with the new place. I've had companies that have treated employees just standard and put in two weeks' notice, was asked for more, but politely declined.
I've worked for shite companies that I literally walked up to my boss, said "I'm done" and walked out the door. If they were really worried about the well-being of their company, they'd treat people good enough that they'd be willing to give them the courtesy of a transition period.
There's still room these days for a company and an employee to have a great mutually respectful relationship. It's just fricking rare.

The last one I resigned from asked for 30 days when i was giving them 2 weeks, the next morning (5:45am), I was met at the gate with a box of my stuff when I came to the plant, wouldn't even let me back inside.
