Bad things would happen. People don't realize the amount of shite on these cargo ships that were just sitting out there during the hurricane. Stuff they use day to day. And heaven forbid one of those fricks crash into a levee.

Every pilot has a commission from the governor. All of the pilots are subject to state regulation via the Pilotage Fee Commission. The commission covers a certain part of the river (also, there are similar situation in Lake Charles as well).
Technically, the pilots are all independent contractors. They band together in the different regions to handle all the support and back-office necessary. Also, it allows a few dispatchers to control the traffic and pilot assignments, as opposed to several hundred pilots trying to do it on their own.
The state board sets the target pilot compensation - this is the amount each pilot would make, gross pay, if every pilot worked the exact same amount. This amount per year is divided up and assessed to all the boats using the services. In addition, there are tarriffs to pay for pilot retirement, and pilot transportation.
All of these fees, taffiffs, etc, are public record.
These guys do work hard. They spent most of their adult life going up and down the river. Is their neoptism? There is, in much the same way that many business owners turn over their business to their children and grandchildren.
The problem is that when the governor gives new commissions, they are mainly based on a list. That list is provided by... the current pilots.
As long as the rates are set by the state, it will be a problem.
