I have a feeling she is the far and away exception and not the rule. Looks to maybe an actress, which obviously have some solid lookers up into their 50s on occasion

Maybe the OP's wife has put all of her stock in her appearance and that's why she's starting to feel inadequate?

It would explain why she married someone 20 years younger. Perhaps she should have been working on her personality and other skills so her self-esteem would be more than just skin-deep.

Chronic insecurity is a tough one. You have to remember that this is a problem that she has and not let it trigger you. It's a personality flaw that the person you chose it marry happens to have. I can tell that you are dedicated to the relationship, keep working on it and ease her towards looking into "marriage therapy" to work on it "together". Any therapist with some good experience will be able to pick up the issue here pretty quickly.

Hey op
How about considering your behaviors?

Do you think your behavior would lead her to feel secure?
Is the mortgage paid on time?
Do you get her cut flowers?
Are you like her father? This is key. Whatever he is about, you are compared, always.

On and on.
Tl;dr
She may be neurotic or craven or crazy but you set the table. You are the trigger.
