Nope you're correct,narcan works to prevent immediate respiratory arrest(which is cause of death in opioid od) but eventually it's effects will wear off and if enough was taken there is the very real possibility they will go back into depressed respiratory drive and eventually arrest.
ETA narcan is near instantaneous and if you push it right the person will come round slowly. if you push it too fast they will become a vomit monster or will try to fight you for ruining their high

That's a hard question to answer just because people define 'clean' several different things. But I stick to alcohol and haven't even really been doing that much since lent.
