Had a front end loader operator run his loader through the plant managers office and drag him out with the bucket, killing him. The loader operator refused to get out of the cab, so cops shot him.
The two were skimming materials and selling it on the side. Plant manager wanted to cut the Operator out.

I used to work a plastics manufacturer that produced food grade containers.
One night I was called in because one of the line workers had diarrhea on the assembly line floor and then proceeded to get almost completely naked and wash himself off in the sink in the restroom.

Someone was blowing up the restroom multiple times a week. They placed signs in the restroom appealing to this persons conscience to no avail. Being a forensics lab they finally sampled one of the chili dippers and identified the DNA and had to have a sit down with the employee in person.
A developer got busted putting early alpha versions of Windows on his own pirate software site. We worked for a large international software company so he was promptly fired.
On a quarterly regional sales meeting at Hilton Head one of the new, young sales reps gets hammered and begins telling the VP of Sales what a dumbass he is. He was let go the next morning, his room and airline ticket was canceled immediately.
