I have only fired three or four people in the past 10 or 15 years. I can tell you, if you were exceedingly difficult, rude to my staff, generally a pain in my butt, as long as I find you alternative care, I have every right to fire you.
If you fire me, it is a blessing. It means you are a Chad and I don’t want to deal with you anyway. I am a surgical subspecialist. I’m booked out two months for routine appointments, and do not need your business.

I would write out exactly what you wrote here on a doctors critique site like yelp so others see it too.
For example, please note I DO NOT live in New York City but out of the blue just typed in "optometrist Manhattan reviews".

Now find your own doc and knock her so others know what she is doing.
I know she is not in Manhattan it was just an example of how to do it you can do it by googling her name and yelp Doctor Jane Doe Optomotrist New Orleans (or whatever).

Be aware that some sites allow retorts to reviews though. Not that any practice may bother. I will say one thing: no one ever regrets firing a client; they just regret not firing them sooner.

MD, DO, or Stem PhD. Otherwise, I'm not impressed in the least. It's total nonsense. It's a good marketing ploy on their part though.
And please explain to me why a Stem PhD gets a pass but PT and Pharm D don't? I don't think one of these is smarter than the other. They take a substantial amount of school.
