re: The cost of Medicare

Posted by CarlArredondo on 2/9/25 at 3:44 pm
CMS reimbursement for physicians is going to be the nail in the coffin for private practice in America. I’m a surgical sub specialist in a poor southern state that is not quite as shitty as Louisiana and reimbursement here is amongst the lowest in the nation. Physicians are absolutely not benefitting from Medicare in any capacity. I currently have to limit the number of Medicare patients I see in a day as reimbursement is a fraction of private insurance here. There are surgeries I do where private insurance pays 3 times as much as Medicare. Medicare pays 20 percent of what private insurance pays for anesthesia for some of these surgeries too. It is total shite and I’m going to retire in my early 50s and do something else.
“And nice moustaches too, Jesus Christ.”

-Bregman quote from end of interview to crews and skenes.
I have only fired three or four people in the past 10 or 15 years. I can tell you, if you were exceedingly difficult, rude to my staff, generally a pain in my butt, as long as I find you alternative care, I have every right to fire you.

If you fire me, it is a blessing. It means you are a Chad and I don’t want to deal with you anyway. I am a surgical subspecialist. I’m booked out two months for routine appointments, and do not need your business.
Sounds like my neighbor who posted a picture on Facebook wearing a mask with her fist raised at a BLM protest. She came home, hopped on her golf cart to ride to our all white country club to be served mimosas by our almost all black country club waitstaff.

I’m 5’ 10” 140 pounds. Too skinny but I was overweight and felt shitty 7 or 8 years ago. I feel good now and labs are great. I’m in my early 40s. I don’t drink M-F. I just eat dinner during the week - whatever I want, no breakfast or lunch. Saturday and Sunday I eat and drink what I want all day. I drink lots of carbonated water. Seems to fill a void that would otherwise be occupied with alcohol. I also try to run 3-4 times a week. It’s become such a habit i don’t even think about it. I saw a picture of myself 7 or 8 years ago with my hot wife and I was so disgusted by my appearance I decided to change. No calorie counting. No formal dieting.
I had whole life, term life, and supplemental disability that was sold to me when I finished training 10 years ago. All with guardian. I canceled all of them when Covid first started. They were actually quite easy to deal with. Give you ample opportunity to change your mind over the ensuing couple of months as well. No pressure on their end. Sent me a big check for the cash value of the whole life as well, it was tax-free as my cost basis was much higher. I switched to just term life, and a different company for disability insurance. Pretty significant savings.

MRI negative. Will wear brace for a week or so.

I wouldn’t run on it if I were you. Chance of tendon rupture is much higher if on steroids and cipro, but cipro alone can do it. I’ve seen bilateral Achilles rupture in a 20 yo swimmer from it.

This is from the actual Pfizer presentation to the FDA advisory committee on 10/26. Basically the study in this age group is too small to know for sure about complications but we will study it after it’s approved. Later on it says that if you vaccinate a million kids, then over a 120 day period you will decrease hospitalizations by 170. Crazy.

I have a 6 yo in a 15k a year private school here in the south. They are partnering with a local pharmacy and holding a vaccine clinic at school for 5-11 yo next week. The teachers were giving rewards to kids for returned permission slips and asking the students if they were going to get vaccinated in front of their friends. Pfizer study on this is completely inadequate. It says nothing about transmissibility, and admits to being too underpowered to show uncommon side effects. Multiple members of the FDA advisory panel thought this should only be given to high risk kids (fatties, asthmatics, cancer etc…) And I’m vaccinated.
