Depends on the state... but I think in LA it's automatic revocation/suspension of your DL

I think you go straight to jail... but have the opportunity to talk yourself out of it in court.
In SC I wanna say refusing a breathalyzer can get you an automatic 6mo suspension but I could be wrong.

About 20 years ago, I had a friend who refused it and was arrested. The outcome amounted to being charged with DWI, a pretty hefty fine, and had to have a breath device installed in his truck for several months.
When he went to meetings in the daytime, he'd park far away from everyone else so he wouldn't be seen blowing into a testing hose to crank his vehicle.
