Guy used steroids for over 20 years, but at least he was very honest about what he took and how. And actually advised guys not to take steroids.

It varies - some guys drift into it from other pursuits - Arnold was a weight lifter (obviously the closest related endeavor) and Columbo was a boxer, for example. By the time of the late '70s, it was drawing folks from all walks of life - it had been marginal through the 60s and early 70s until the Weiders decided to push Arnold to stardom, with the help of the documentary Pumping Iron.
On the male side, it's a lot of skinny kids who got picked on (late bloomers), coupled with athletes who lifted weights (football players and weightlifters) who screen themselves in as they've already lifted a bunch of weights and gotten good initial results.
But, the drugs on the elite professional level have been monstrous for well over 40 years. Bill Pearl talked about steroids coming in the 1960s.
As a crazy mind frick - Sean Connery was a 1950s era "bodybuilder" competing in Mr. Universe in 1950s when they looked like this:

Larry Scott, for example, was primarily a bodybuilder from age 16.
Gets pretty strange after that.

I read an interesting piece one time that claimed NFL players started using roids back in the mid 70s. Mainly to help with recovery after games. But not widely used or known about until the late 80s.

The Steelers offensive linemen from the 1970s are considered the NFL steroid "pioneers" - probably drifted over from bodybuilding to them, too.
It created some friction in the lockerroom - defensive guys like Lambert and Greene were natural training advocates and (rightfully) thought that the drugs were dangerous, cheating or both. Almost all the OL were juicing, especially Courson and Webster. Webster was the strongest man in the NFL before he started juicing. He became a literal monster of a physical specimen.
But at what cost (of course the collisions required of the position don't help)?
