Same here. I turn 40 this year as well. I do it for myself and for my family.

I had hemorrhoids that were like marbles hanging out my arse. Tried all the topical stuff for a week or more and they never went away. Finally bit the bullet and went to a proctologist. I pictured it being worse - thought I'd be in stirrups or something, but they just laid me on my side and lifted a cheek. All done with local anesthesia, and the shots in the 'roids don't feel pleasant. He was very into the procedure. He was describing it in detail, telling me how he was making incisions and squeezing stuff that looks like raspberry jelly out, and even showed me the stuff on a piece of gauze (not by my request). The part that sucked is that I thought I would walk out of there with a normal arse. No such luck. Takes days for everything to go back to normal so I still had marbles hanging out my arse when I left. I was supposed to do a lot of laying around, take it easy, etc. Instead I went and raced motocross the next night. I brought vaseline with me and was in those nasty port-a-johns at the track lubing up my a-hole all night long. You're supposed to stand up a lot when racing motocross, but most of the older lazy fricks like me would sit most of the race. On that night my arse never touched the seat, and I made it through both motos and trophied. I still consider that 'roid race to be one of my greatest accomplishments in life.

This is a non-emergent reason to schedule an appointment with your (friend's) primary care physician at your (friend's..,just assume this if I say "your" again) earliest convenience. Let me clarify by saying "your earliest convenience" should be sometime within the next 2 weeks if at all possible (no reason to go to ER, but definitely not worth waiting on).

Here's what's going to happen:
Friend gets a small blood draw to test for anemia (will determine chronicity-ishness) and will get a finger in the rectum (trust me, worth it). That finger will either feel a hemorrhoid or not. Either way, it'll probably be swiped against a card to determine whether or not heme (blood) is present). From there, based on the findings (+ answers to a few questions), you'll get something between a recommendation for dietary change (more fiber, i would suggest Quaker instant oatmeal as a great source here) to a scheduled colonoscopy. Hemorrhoid surgery probably won't be the end-all IF this is the first occurrence, stooling (/pooping) isn't hindered (only takes a few drops of blood to turn the bowl pink), and dietary + medication (HTN in particular) changes don't stop the problem WHILE ALSO displaying signs/symptoms of anemia.

Hope this helps. Will try to clear things I said up tonight/tomorrow if you question any of them. Best he/she can do between now and Doc appointment is increase fiber intake. If it goes away, it's still worth a visit. You'll still get a finger in the rectum and a blood draw, but at that point, they may hold off on everything else if s/a go away.
