It kind of is. Because, like I stated earlier, if a government paid for your $2k ambulance ride, when it could have been a $30 uber ride, somebody has to pay for that.
They have to clean the cab, anything they stuck on you ( which they're required to do for BP and other monitoring, etc.).
Someone paid for your ride. Even if it's me overpaying for an X Ray to compensate for what you're not paying for 2-3 people, not including ER staff, to deal with your arse in a bus that could have spent $30 to get to normal intake.

If he’s stupid enough to not get his leg looked at for 3 days and which resulted in higher medical costs borne by those paying premiums in his group, you think he is smart enough to understand this?

I mean, if my knee swelled or locked up I would just assume I strained it somehow or maybe arthritis. Deep joint bacterial infections are fairly rare and I bet most people just assume they are getting old for several days before the "oh crap I need to go to the ER" moment hits.

Over the last 2 years my right knee has locked up like that on me twice. The pain is UNBELIEVABLE, with crutches I don't know how I was able to get to the bathroom, but did. Went to drs/hospital and no real diagnosis. They told me about the arthritis shown by x rays which I have from head to toe. It lasted about a month each time- steroid shot and Voltaren was their treatment. About 15 yrs ago it was my back ,I couldn't move and had to call an ambulance and back then they were dumbfounded as well, just kept me on a morphine drip for a while and watched my heart and other issues

It was almost three years ago to the day. My leg had been hurting, but I didn’t go to the hospital at first. When I finally did, they just gave me pain medication and sent me home. A few days later, the pain got much worse, so I went to the VA hospital in D.C. By the time I arrived, I collapsed in the emergency room.
I ended up staying there for almost three months due to an infection they were never able to diagnose. They eventually performed a procedure to drain the bone marrow from my femur and replace it with antibiotics. The damage that had already been done left me unable to walk for over a year afterward.
Today, you wouldn’t notice anything if you saw me walking, but I still can only bend my leg to about 65%.
I ended up staying there for almost three months due to an infection they were never able to diagnose. I had morphine but for some procedures they actually did give me fentanyl
