Assuming that Isabella in the Black Rock was not the MIB, we are left with wondering who it was. I can only think she was:
a) just a hallucination because Richard was so dehydrated and traumatized
b) Jacob - some have said that he may be able to take the form of others, ala Christian Shephard with white shoes
c) Some other being that we don't know about?
Hurley was able to see her and speak to her so I guess that places her "ghost" on the island. If Isabella could appear on the island later I would think that she would be able to appear to Richard while he was still in the Black Rock.

I do have a question about dead people appearing and dreams.
We have two instances of them happening that I remember. The first was Christian seeing Shannon and the other was Locke seeing Horace cutting down the tree with a nose bleed in "Cabin Fever." The nose bleed indicates to me that he's flashing through time. However, Horace tells Locke he's dead. Then the scene starts repeating and the tree reappears and Horace acts as if he's seeing John again for the first time.
Who is manipulating dreams? That might be insignificant in the grand scheme of things but it seems pretty relevant since it causes action on the part of the character, especially the Horace dream.

He was involved through Richard

Don't know, but I'm still going with my theory. Why? Because it's mine.

well we know Richard went back in time to visit a young John Locke when he was like 5 years old. somehow he got off the island. I think there are other instances of him doing this too but I haven't seen all the old episodes.
He didn't go back in time.
Locke came into their camp in the 1950s and told them he was destined to be their leader. He also gave Richard a compass and told him the year he was born.
Richard left the island five years after John's birth to find him.
Richard has also been off the island to recruit Juliet.
I'm not sure I'm remembering this correctly, but wasn't he also standing outside the delivery room when Locke was born? OML?

I thought this too.
I also think that he could have been dreaming. He falls asleep right before this happens and after the seen, he is asleep again.

Also, how do you explain the canidates on the ceiling?
They're playing the same game.
MIB knows who the candidates are.

--------------------------------------------------------------------------------
Ben knew also early on. He had a list of people for Michael to bring to Dharmaville. That was the list of candidates that Ben got from Jacob or Richard.

People keep saying this and I have one small doubt that it was the MIB, since we have never seen the MIB take both the form of the smoke and the form of a human at the same time. While Isabella was running up the stairs in the Black Rock, you could clearly see the Smoke Monster. Maybe I am reading to much into this, but it seemed odd to me.
They have shown Smokie split and converge from three different smoke blobs (I think in season 1). So they have hinted at this already.

He didn't go back in time. Locke told Richard in 1954 (?) when and where Locke would be born, to verify he was their leader. So Richard remembered and checked it out, then watched Locke as he grew up to see if Locke was "special". Hence the visit by Richard when Locke was a kid.
Richard experienced all of that linearly.

How would Richard do this? He didn't have the sub and the black rock wasn't exactly ready to sail so how would you think he could bring people to the island when he seemingly couldn't leave the island?
Richard didn't. Jacob brought them their just like everyone else. It was Richard's job, after they got there, to be Jacob's spokesman. Once the sub arrived, and stayed, they used that to retreive specific people.

Who were the first groups of Losties whom the others kidnapped? Were these just people whom they wanted to join them as others?
Candidates to replace Jacob and candidates to become others?
